Cassette worn or dirty?

High gears (small sprockets) look flogged out which should result in jumping in gear, which may seem like shifting issues. New cassette and chain in order. Service your cables aswell as poor shifting can be cable related.

No, they're fine. The combination of the shifting tooth profiles and the tight sprocket radius can make the small gears look badly worn, when in fact they're usually the least worn of the lot. The 3rd & 5th largest (and presumably the 4th, but can't tell because the chain is on it) sprockets are showing a bit of wear, but not excessive. The chain has seen better days and should be replaced ASAP. The current degree of sprocket wear should be OK with a new chain.

As mentioned above, check the cables; they do stretch over time, which degrades the shifting, and depending on quality can corrode, which will incrrease friction and also degrade the shifting.

Much as what Duck has said, I'd probably only add that the rear derailleur hanger should be checked for tightness onto the frame mounting point, and redo the cable/cable housing...also that chain looks loosey-goosey but I think someone already said that.
