Good physio in Brisbane?
Postby Queestce » Fri Sep 09, 2011 4:46 pm
I'm having a little trouble with my left knee and although it's not an issue as yet, I sense it getting worse before it gets better unless I sort it out. I'm hoping to make an appointment with a physio in Brisbane but figured I'd check if anyone on here could reccomend any? Ideally someone with a good knowledge of cycling would be helpful as I'm fairly sure increased riding is the cause! I'm located in New Farm so anywhere close to the CBD would also be easier, but I can travel if it's worth it Any insight would be much appreciated!
Cheers,
Tk

+1 for David from Cycle Physio. Great for a bike setup and ongoing body/physio maintenance.Comedian wrote:Cyclephysio.com.au is good but not centrally located.

- hip asymmetry
- femoral, tibial, fibula length and torsional asymmetry
- ankle and sub talar joint range of motion and degenerative disease.
- forefoot varus/valgus
If so, does he do it with x-rays, or another way?
Does he have a view on orthotics with metatarsal domes?
Did he experiment with cleat vs in-the-shoe wedges, vs leg length shims, vs pedal spacers, vs crank length?
I understand his focus is more on core strength. Can anyone give insight into his recommended exercises for core strength, or stretches for lower limbs, spine, and upper limbs; relaxation for neck?

Mate... I'm an IT geek. David took lots of measurements. I can't tell you all the !! BAN ME NOW FOR SWEARING !! he measured. Actually about the only thing he didn't measure I definitely can't tell you about.winstonw wrote:Can anyone tell me if David measures

What I can tell you for sure is there were no x-rays involved... he thought metatarsal domes were really good.. he measured my foot thingy and told me to get some wedges (I love sour creme and chilli!).

As for your first question, he does measure everything, though not using any explicit x-rays taken for the setup process. I'm an above knee amputee with alot of asymmetry (back/hips/leg lengths) and we took quite a while dialling in my setup (which he's done a great job at )winstonw wrote:Can anyone tell me if David measures

Secondly, he has given me a number of stretches to do before and after riding, specific to my needs. The only way for you to get an real insight to his recommended exercises is to pay him a visit.
Good luck with it.

The reason for the 1000 questions is because I've seen two specialist bike fit physios already; and there was a lot of stuff I felt wasn't covered. They also contradicted each other on several issues.

no surprises there. bike fitting is not a science, but more an art, based on experience (and some science / physiology) - just ask steve hogg. anyway, winstonw wrote:Thanks for feedback guys. I had a few wines under the belt when I wrote that post above.

now in messy point form.
- measuring hip assymetry is subjective. so many factors can make you pelvis seem uneven - and they're rarely pelvic.
- tibial and fibula length - just one will do, but we'll call it lower leg length, as its typically from knee joint line to that bump on the outside of your ankle.
- torsional assymetry - do your feet point at at the same angle.
- sub talar joint range - no idea why you'd care about this and i doubt anyone could measure it in a meaningful way.
- hard to measure degenerative disease except via post mortem
- forefoot valgus/varus. again, meaningful measurement is hard to attain. often rearfoot valgus is used for normal people, but us cyclists tend to be more 'on our toes' .
met domes are good, maybe even orthotics for the mid foot - though there's no need to worry with the heel that much.
and speedplay pedals - then you can shim / wedge / change axle length and have all the ankle float you need to be in more control of your lower limb cycling biomechanics...

and I wonder about the science. The first guy was adamant I couldn't get more hip flexion unless I stretched my hamstrings.scotto wrote:no surprises there. bike fitting is not a science, but more an art, based on experience (and some science / physiology)
The second reckons it isn't the hams that limit hip flexion in non overweight cyclists at least, because the muscle crosses two joints, and when the hip is flexing the knees are also, thus keeping the muscle slack enough not to restrict hip flexion. He also said the primary issue compromising a more aero position and stronger hip flexor engagement isn't a lack of hip flexion, but a lack of lumbar spine flexion.
Another point of disagreement related to my lateral knee excursion between 10 and 12 o'clock. One said it is due to foot alignment issues, and the other due to tight glut max and other butt muscles, but not hamstrings.
