When you say inside, what do you mean? Interior of the joint? left side of right knee or right side of left knee? rear of knee? More detail would help.

That said, nurofen and a wetsuit rubber knee support would be wise, followed by a trip to the physio as soon as possible to get strength/stability exercises underway. Staying off the bike unti lyou get to the physio would be prudent.

MIght be a smart idea to get it diagnosed properly before you start working on exercises. I would stop cycling right now if were you. YOu may be just making it much worse.

You've probably damaged a ligament or tendon. You need to find out how long it takes to heal.

I damaged knee ligaments in running once. was advised to give up running for about a year. Its really not worth messing around with.

Sorry but I have no time for complaints about specific pains in specific places. I get pains all over my body.

But seriously, a word of advice from someone who has contributed to his own physical problems later in life - seek professional advice early and do not be in a hurry in any matters to do with your physical well being.

OK, that's more useful info, and less worrisome. :D

I'd still go the physio, but perhaps a cycling-specific one. Out of curiosity, do you also pedal knees-inward a little, perhaps brush your top tube on the upstroke from time to time?

Sounds like you are similar to me then, and have a varus forefoot where the big toe knuckle is higher than the little toe, leading you to tend to roll your foot in slightly. This will lead to the type of knee pain you (and I) have experienced on the bike. For me, it also contributes to sacro-illiac joint pain.

I've found the Specialized Body Geometry insoles very helpful in alleviating the problem. Running both sets of orange varus forefoot shims stops the knock-knees and my knees are much better aligned between ankle and pelvis - and now hinge in the correct plane during the pedal stroke.

For normal walking I have orthotics that tip my heel outwards so that I don't pronate.

You've reminded me I need ot resume my core and leg stability maintenance exercises now that the flu I had is gone.

If you can reproduce the pain by pressing on the medial collateral ligament a little above and below the joint line, then you likely sprained it.
If you can reproduce the pain by bending your knee and pressing just in front of the MCL, into and downwards in the joint space, then it likely includes a medial meniscus insult.
You might have hurt both structures too.

The former can heal faster than the latter.
Both would benefit from rest from vigorous activity, ice, appn of support bandage, elevation at work, oral anti-inflammatories, hydration, .....gradual return to activity as pain settles.

I occasionally get pain in the same place, if I run or pedal in too high a gear while going up hill. The physio says it is bursitis, and results from a large Q angle.

+1 for rest + referral.

Maybe check your seat height too. It could be a little low if you're getting pain at the top of the upstroke. This might be causing your knee to bend a little more than it should.

Yours is from repetitive stress. According to Dave, his isn't.
