Go! Alliance in conjunction with BNA is looking to define reporting categories that cyclists and pedestrians would most often use to report something not right on our roads and paths to each other and to the respective authorities. Those items that you would want to hear about or want to alert others to be aware and to get fixed.
We are looking to build these into an application that is under development... but right now tell us your top 10 categories that would be useful and meaningful to you? Assume that these would all be automatically geo-tagged from your smart phone or inidcated on an online map. A description box and photo utility will be included to further detail the issue.
Okay, here's a starting point - use these or make up new ones for your "Top 10"...
1. Obstacle in path
2. Broken glass
3. Abandoned bicycle
4. Bike rack request
6. Signal problem
7. Broken pavement
8. Police patrol request
9. Improved lighting
10. Signage request
11. Black Spot - use caution
12. Ramp request
13. Elevator request
14 Bike lock device (a tag) with refundable deposit. For leaving your bike at a set of racks.
I think you'll find that black spot will be a commonly used one and you can sub-categorise that with 7, 8, 9, 10, 12 and 13, since all of those require a reason to exist. In other words, you, as a sensible cyclist, find a problem on your journey and decide how it could be solved. You then tag a black spot and indicate that a ramp would be useful here. If we don't tell them what we want, we'll never get it.
May I also suggest including an accident assist feature with this app. They have similar things for car accidents and, provided you're ambulant, you could be prompted to gather appropriate information.
Having a big red panic button might also be useful, something that you can hit and the phone would (depending on preferences) geo-tag, start videoing and voice recording, so you can record an incident quickly without having to start lots of apps with the adrenaline flowing. Uploading to the cloud might also be a good idea to preserve evidence. Tagging and reporting can occur later.
You might also like to map these situations/incidents so that others on similar routes can be warned in advance and possibly even contribute comments and further evidence. They could even up or down vote a suggestion depending on their experiences with it.
Those are great inputs, thanks! The trick is in the balance between too much detail and not enough in the prescribed categories. Too many sub menus can get tiresome. I like your assist suggestion and also the video... we anticipate a video feature to be included at least at the reporting level. The geo-tagging is automatic with GPS enabled phones. No more than 3 clicks to do anything is the general rule. Stop, pull phone out of back pocket or off of handlebars. BikeTagApp -> Blackspot -> Need a ramp (auto photo?, geotag). Keep riding. Steve Jobs would approve.
Definitely accidents/incidents. All largely un/under reported and completely destroying any hope of gaining useful stats for anyone's case for safer roads.
Clean lane/path/road request (for lanes and path filled with debris)
Fix lane/path/road (eg. potholes and damage)
Improve infrastructure Request
Improve Signage Request
There could be more though I see a difference between request which are more urgent, such as cleaning a lane/road or fixing. Improving infrastructure and signage could be a type of tally - possibly goal associated inwhich the (cycling)community is motivated to create support initiative to improve facilities.

The Go! Fix App has now been released - BNA is a supporting partner and with massive promotion directly within the cycling communities, this will become the default app for cyclists to report issues to their local councils.
