Acute epididymo-orchitis AKA Painfull testis

This is a story of pain, misery and sheer frustration!
‘Twas the night before the Strava Rapha Rising challenge (13th July) weeks of training saw me in good shape to complete the challenge alongside my friends and other local riders, idrycles was even going to come along and show me up! But before the hill climbing commenced I had to appease the minister of war and finance (Pam) and do a social ride, so the MTB’s were loaded in the car along with other biking paraphernalia, for an early morning Parklands trail ride when suddenly sudden pain in the testis had me feeling rather unwell.

Thinking I may have bumped them when loading the car I waited about three hours before I finally gave up and went to the local ER. I was rushed straight from triage to the ER for immediate assessment. This was fortunate as I did not wish to be sitting amongst people who are allegedly ill, but well enough to be tucked into pizzas, curries and other take away food, what’s wrong with these people, honestly. I know I would not be eating in an emergency room, who knows what others are coughing up or have left behind, I digress.

Initial thoughts went to a twisted testis and they wanted to operate straight away and I had to sign a permission form and initial next to what was literally a release form, as they may have needed to turn me into a Womble! (That’s phonetically spelt)

Luckily for me an ultrasound specialist was on her way home after sorting out an earlier emergency and stayed behind to have a look. It seemed my testicle thankfully was not twisted, but it had developed a case of acute epididymo-orchitis http://goo.gl/s81KZS . Now I have had epididymitis five times in the last seven years, my GP thinks its caused by back pressure due to my vasectomy, whatever that means. The main causes of my condition per the article (the sexual ones) have been scrubbed off the list, I don’t partake in one and the other has been tested for and cleared. The infection is normally treated by strong antibiotics for about ten days and it all goes away, normally permanently, but for me only for about 12-14 months.

After spending the weekend in hospital with IV drips and other things the Hospital urologist did his walk around on the Monday and got quite concerned that after four other attacks this had not been addressed before and suggested an epididymectomy and to be further consulted after discharge. I was sent home Monday 15th on something called open leave as they needed the bed, just returning for the next two nights for more IV anti-biotic. Prescribed oral AB’s twice daily for the next four weeks and a told to abstain from any exercise cycling or otherwise and alcohol for four weeks. This was not good news particularly the cycling bit.

I had a place in the Tour De Cure 4 day QLD leg, a once in a lifetime opportunity this has had to be cancelled along with Tough Mudder, Sunshine Coast 10k charity run and as mentioned earlier Rapha Rising. Several events coming up, including wild horse mountain, the Ewen Maddock adventure race and some mini triathlons (my first attempts) I have been told to cancel / not register for too. Not happy.

My GP wanted me to give up cycling and find another hobby and thought that castration was the only option :shock: , thankfully I had read about what happened and learned a bit from DR Google and mentioned what the hospital urologist had said too, he just signed me fit for work and said that after the AB’s if it was not better to go back and see him.
After about seven days I was feeling awful and cold/flu symptoms and my body was clicking and popping at the neck, shoulders, elbows, hips, knees and ankles. Spoke to the Dr and hurrah, a drug side effect, you now have tendonitis too!

So the AB’s were changed to a new drug, the tablets are that big (1000mg) I seriously wondered which end to put them in. Thankfully all seems to be ok with these drugs, but here I am nearly three weeks on and still in a two to four pain level zone, no exercise, no nookie, I have a sit down desk job and yet still it throbs. I have been taking anti-inflammatories and pain killers (I am not a pill popper, but this is crazy) I am re-appointed today at my own request with the doctor to be referred to a specialist to get this fixed once and for all.

I have pre-empted the wait in the Medicare system and been quoted operation prices and thankfully if as suspected the procedure will be about $1000, affordable. (Rather spend it on a CX bike to be honest)

I think most jokes about swelling and balls have been made, but happy to hear more! Good for the spirit. But in all seriousness has anyone else had/ suffers with this? PM if you want, I understand if you don’t want it public. Anyone had the op, what was recovery time?

Apologies for the long winded opening, but at least, when you have finished wincing you can smile and think, phew glad it’s not me!
Is anyone interested in progress or should we just leave it at that….. 8)

Update,

Well back to the hospital for our patients yesterday. Up until about a week ago I was ready to resume exercise but thought it best to seek permission.
Then without warning the pain was back, but it felt more from the groin and intestine, HERNIA? nope, had them both repaired in the past and the doctor checked me out for that only four weeks ago and again the other day.
I have been sent for a CT scan of the abdomen and groin to see what is going off, I will be back in hospital later this week for another test. The doctor feels its referred pain from the lower back.
Spoke to my dad last night and he suffers the same, exactly the same. It feels like a hefty kick in the b*ll* followed by someone trying to pull your stomach through the side of your groin, he said. I agree. He suffers with a collapsing spine and has already had three hip replacements, he is only 63.

I am waiting to hear the results still but in the meantime, is it safe to ignore the pain of referred pain and just go for it? The reason for my urgency is I have just received an amber light from the QLD police about my academy application. I need to hook back into the fitness quickly and go,go,go!

How about a saddle with the centre hollowed out? I had a similar complaint (though sincerely less acute) I got a new seat and have a lot less uncomfort now.

Ouch. I've had epididymitis apparently due to back-pressure, but thankfully it only lasted a couple of days. I thought I was in pain until I read your initial post...

Apparently (and this would have been nice to know BEFORE I had a vasectomy) epididymitis is fairly common, with about 1/4 of cases so severe that they do a reversal. 1/8 of those having a reversal still have issues...

100% with you. WHY did NO ONE mention this could happen.
This was my 5th attack, normally like you, I get a bit sensitive and take a trip to the DR and get AB's for 10days. But this was different.
Can anybody shed light on the BACKPRESSURE bit.
Funnily enough I was at the hospital yesterday at the request of the head of Urology, I was not allowed home till he had seen me. All is clear once again, he has written to my GP and I am to be immediately referred to him if I have another attack, he said it was simply "not fair"to keep suffering this on an almost yearly basis.

I missed this thread earlier, - good ole post vasectomy syndrome wish I had known about it before too. Mind you I appear to have grown out of it (15 years later :shock: )

They stop up the end of the tubes. Usually the tadpoles die and are reabsorbed quickly enough to not cause discomfort. However you get spikes of production every now and then that create a backlog and start stretching the tubes. AB wouldn't do much unless you've got an infection. I wouldn't think they'd be that common.

Mine has always been treated as an infection! That's interesting information that....
I know my last episode was an infection as I was Iv'd and had a fever too!

I have two "Prologue", one "Road", I won't ever go back to a "standard" saddle.
Prologue gives you the biggest range of saddle positions.
I ride, therefore I am. But don't ride into harm's way.
...real cyclists don't have squeaky chains...

Been having an email chat with Dave Schindler from ISM and he too recommends the "prologue"I questioned the saddle width as I am 143 at the moment.
His subsequent replies assure me it will be fine and offer a variety of positions. I will be placing an order, price shopping now..

Purchased a ISM Race, wonderful invention. Initail discomfort as everyone says from sit bones, but everything else is great.

The saddle was obviously not the issue for the testical trouble, I have two urologists telling me different things, one that this is almost certainly an infection, the other telling me that I am over virile and suffering from pressure leakage, blowback as he called it :shock: and irritation from the vasectomy!

Either way it still flaming hurts when irritated.
I have been chomping the pills which has lead to other complications and a feeling of illness overall. See http://goo.gl/DAetzV
but since then I have reduced my pill intake, I much prefer niggling pain to how I was feeling before, I still think I am detoxing though.

So going through the wars as one kind soul put it, for those who do not know me I must sound like a walking wreck or a hypocondriac, it's not going quite to plan at the moment with my health!

Just been placed on Hiprex AB's for the foreseeable future, will let you know how it all goes.

I hope all this helps someone else not suffer like me for the last 6 years and really suffer for the last 12months!

long ago riding caused me to have a twisted teste and i went under the knife. they fixed it up and AFAIK i'm still firing out of both barrels.

a mate of mine had similar pain and it turned out to be his gall bladder causing infections. they cut it out and he was fine after that.
