Was interesting watching it in french and trying to figure out where O'Grady came from...and then watching it in English and having the commentators ponder the same thing!

And it was all caused by a loose water bottle and the slippery patch on the cobbles it caused. I also understand that it happened on a steep cobbled downhill.
