I would suggest making the arms tilt slightly upwards, rather than downwards. Have thumbs pointing straight ahead (and the wrist/hand position that follows doing that), thumbs down or up is slightly less aero, although you may be constrained by what's comfortable and the available equipment.
I'd want to know the distance from centre of BB to end of extensions to ensure it's legal, or if beyond 75cm, that it's no more than 80cm and your elbow angle is < 120 degrees. Having the slightly upwards facing forearems actually helps that a bit. ear-elbow position looks OK.
