Thanks for sharing that, good article and good links.
The dangers of car doors opening needs repeated emphasis, it makes me shudder to see beginner cyclists ride at speed, close to parked cars.

Yes I know someone who's brother was killed that way - I' ve never forgotten it and it's kept me at a safe distance.

Dooring from the right and pedestrians (treating the path as a staging zone) are the reasons i'm amibivalent about Melbourne's "Copenhagen" style bike lanes.

My previous preference was to stay in the line of traffic when stopped. Especially behind left turners (unless it's obvious pedestrians crossing are going to hold them up). Lately, i've found it confuses some drivers who don't believe I'm not suddenly going to dart around them. We also seem to have scored a few traffic lights which give bikes a head start. That makes you feel silly sitting a few cars back...
