Could we also generate a template for a post-crash situation for riders to keep in their saddle bag? At the very least, should an accident be fatal, the rider will have emergency information ready. I believe this should be vital practice for all riders:

1. Stop in a safe place
2. Call an Ambulance for anyone who is injured
3. Phone the Police
At this time, the Police do not care about who was at fault so prepare to remain calm and tell them clearly your recollection of the event. Be careful with your words. If its a minor accident it can be reported at a Police station later in the day.
Document Contact Information of any witnesses including Rego, Name and Phone Number:
Time of Incident:
Swap your details:
Make, model and colour of the Motor Vehicles involved
Capture as many photos of the above details including the context of the accident as possible. Video is preferred.
Local Police in most states: 131 444
Hopefully a rider would never have to use this, but a lot of things can be forgotten after an accident. Worst case scenario, you take this straight to the police after the crash to document the accident.

I'm about to file a report for the February 13 left hook, and was wondering whether you guys put your contact number in the letter or do you leave it off to force a written response?

Absolutely, put all contact details. Then when the front desk droid fails to forward a written and printed statement to the investigating officer you can drop them in it. That's what happened to me (see SA forum topics) when a bus driver tried it on. Police officer rang me to 'come in and I'll take a statement' and I said 'But you've got the one I handed in at the time, yeah?'
How long should it take for police to do there job once a complaint has been made?
It has been a few months since I put in a complaint and I have heard nothing.
Emailed him once, no reply.

Quick query for you guys, when submitting multiple complaints via the template do you submit one copy of the main letterhead and then refer to the multiple complaints as attachments, or do you submit one copy of the whole document for each report?

I would recommend doing each report separately - if the drivers are charged there will need to be a separate file & no point giving the driver the heads up about other offenders

It includes a field for "social media links" including youtube.
I know it specifically says "hoon" but the preceding page ( https://www.qld.gov.au/transport/safety ... index.html ) includes "Driving recklessly or deliberately endangering other road users" and "Blatant disregard of road rules and signs"
I would like to invite members to create a new template for the benefit of all. I can add this to the top, and of course the person who creates it will be credited / acknowledged.
Perhaps it will be useful to create a type of GNU license - i.e. free for all which will solve this whole problem and be beneficial for all cyclists.
